ACL reconstruction is one of the longest rehabilitation programmes in musculoskeletal physiotherapy, and one where the quality of the rehabilitation determines the outcome more than the surgery does.

The early phase

Getting the knee fully straight, controlling swelling, and waking up the quadriceps, which switches off remarkably effectively after this operation. Quadriceps strength lost in the first weeks takes months to recover, so this phase is more important than it feels.

The middle phase

Progressive strengthening of the whole leg, restoring normal walking and then running mechanics, and building the single leg control that the knee will need. This is the longest stretch and the one where people lose motivation, because the knee feels fine and the work continues.

Return to sport

Here is the part that matters most, and the part most commonly got wrong. Returning on a timescale rather than on measured criteria is associated with higher re-rupture rates. A knee at nine months that has not regained quadriceps strength comparable to the other side is not ready, whatever the calendar says and however well it feels.

So we measure. Strength side to side, a battery of hop and jump tests, landing and cutting control, and how all of that holds up once you are fatigued. Those results set the pace, and we will tell you honestly when you are not there yet.

Most guidance points towards nine to twelve months before return to pivoting sport, and often longer for younger athletes, where re-injury risk is highest.

We follow your surgeon's protocol and will write to them. We are also happy to speak to your coach or club physio, because availability pressure is real and a shared plan prevents a good deal of trouble.

    I could not recommend Healthflex enough. I had an ACL reconstruction in August 2020 and I was referred to Martin Dyer by my surgeon. I can see why! Martin could not have done more to get me back up and moving right the way through lockdown. It is 7 months and he has already got my doing a bit of jogging. Cycling is a piece of cake. I am so grateful to all he has done for me as I was concerned I would never get back on the road. He has shown me anything is possible. Big thanks. I could not have done it without him.
